The ''quack'' is present a few times throughout some performances, not just before '39.
Just Roger having a bit of a laugh I think, no underlying meaning.
I'm also pretty sure he didn't do it EVERY time before '39, who told you that?

There was an unpleasant misunderstanding about Brian's education. Roger thought he was a med-school drop out who was practicing nonetheless, so he called him a quack.
