Does anybody know about the existance of this cd-single?

Who wants to live forever cd single with the original cover, not the one from 1992, seen on Pinterest, does it really exist?

It's not from 1986, but it does look like it exists. Maybe somebody somewhere used the original artwork for a reissue?

First time I've seen it. I find it odd that it says "5 inch maxi...". Why would they emblazon that right across? Not sure of this one....
