I have some where a picture of RT autograph on a pair of womans panties, I know it is real, as the woman also send me pictures of her getting them autographed by him( no she was not wearing them at the time...sadly!)and it was during the Game tour, USA

Right elbow I believe, as seen by the extra cuff on his sleeve in the Scandal video (the guitar solo was filmed later when he had healed)

right arm I think..this could solve the mistery of the strange "B"..I have two items from that year and both have perfect band signature except Brian's"B" looking strange:
