Also.. it was probably hard for him to smile cos of all the pain he was going through and knowing that it could only get worse not better (other than the fact that when his death finally came he would be relieved from his pain).

Not really cruel..I'd say sad.
If I knew I was dying I would smile the whole day. Just to be happy the last days of my life :D
