Sorry this probably sounds really stupid, but the first ones look like the ones that you make on the sims 2 website :S... but ignore me. Im insane and will shut up now.

It goes a bit beyond that. I also use photoshop to my advantage, such as hue, shape (warp tool!), of course cutting, pasting, and composting, softening and sharpening. Plus I resize it. I also add stuff here and there.
