I have created a merge of the best "publicly available" sources, it was complete and all sources except 1 were in really excellent quality.
Well, I wouldn't call any of them excellent quality since Freddie sounds like he's singing through an open door from another room somewhere down the corridor. But that appears to be a fault on the master, it's certainly present on Pop Spectacular which is sourced from a BBC Transcription disc.

I seem to recall they turned him down due to his singing sharp (nerves) although the actual mix sounds far from official. A back up monitor mix that ended up on the transcription disc after a cock up? Stranger things have happened!

It is certainly possible for a CD to contain quad coding, but I very much doubt the On Air release will.

The first US broadcast of this material was on WMMS 100.7 Cleveland on March 2, 1975, as advertised here:

With the daring nature of FM radio in the early to mid 70s, it may well be a case of the band not even being aware of the broadcast.
This is likely the broadcast that led to the swath of bootlegs beginning with the "Rogues And Scandals" LP a year or two later - an American bootleg.
Here is the intro by stateside BBC correspondent Brian Saxton - it's been sitting here (http://www.surrounddiscography.com/quaddisc/quadradi.htm) for ages and nobody seemed to notice:
