Making of ANATO

I was re-watching the Making of ANATO DVD today after a long time
and I had always had a doubt about something in it. In the special
features session when Brian is showing off the demo to his arrangement
to God Save the Queen, he tells about E.R. being the producer and
that this is an obvious joke.

Not that obvious to me. Do you know "who" or what is the quoted E.R. in the God Save the Queen sheet?

the clue is in the song title....

someone, anyone please ....tell him/her, can't bring myself to say anymore than google "E.R. queen"

Of course Elizabeth was in my mind, but Regina wasn't obvious to me. Not being british excuses me for not knowing the Queen's initials?

Dear, by initials, I meant what the Queen uses to sign court
documents and in whathever the E.R. appears as a british mark.
Of course I know her name is not Elizabeth Regina, if you thought
so.
