Well, if we're to be serious...
I think any death in which a person is aware and actively dying is an awful death. A prolonged illness, suffocating, bleeding out, starving...
For me, drowning would be the worst way to sign out. I have a strange and irrational fear of water - and though I know it's irrational, I can't get past it - even seeing the word "drowning" in your original list made me panic. So that would be it for me.
I mean, think about it - you're submerged, you can't hear, you can't scream, you can't see. You can't pound on anything to get attention. You can't breathe... yet eventually you will, you'll be forced to, and you'll suck in lungs-full of water, which means you'll basically be killing yourself even when what you want most to do is survive. So... yeah. Drowning.

Thanks to Costa86 I've tracked down a copy of a book about the Hiroshi Ouchi. I know there are great books out there (in scope, Not for joy in content) regarding atomic energy, bombs etc. Just hadn't taken the time to read em.
