I don't think he 'looked like a girl' as you put it. The way he looked suited the song perfectly. And as Roger says it was a good disguise to hide the fact that Freddie was very ill then. And all the things in it like the 'claws' and bananas in the video signify madness which the song is evidently supposed to mean.
