*Dementia is a condition that leads to the loss of intellectual abilities such as memory, judgment, and abstract thinking. It can also cause changes in personality. AIDS Dementia Complex (or ADC) is a type of dementia that occurs in advanced stages of AIDS (acquired immunodeficiency syndrome). AIDS is a later stage of HIV (human immunodeficiency virus) infection.
ADC leads to a diverse group of symptoms that affect your ability to function in work and life. It can be fatal. Before HIV medications called HAART were available, ADC occurred more frequently. Today, only about 10% to 15% of people with AIDS develop AIDS dementia.
What Is AIDS Dementia Complex? (ADC)
Unlike many other HIV-related problems, ADC is not caused by an opportunistic infection, which takes advantage of a weakened immune system. HIV experts say that ADC results directly from the HIV infection itself. Although HIV does not infect brain nerve cells, the virus may indirectly inflame them or kill them. This is more likely to occur as CD4 cell counts decrease to less than 200. CD4 cells are a type of immune system cell. They are also known as T-helper cells.
ADC varies greatly from person to person. Symptoms may progress quickly or slowly. It generally affects four different areas of your brain function:
Thinking abilities
Behavior
Coordination and movement
Mood
Symptoms of AIDS Dementia Complex
The signs and symptoms of ADC can be similar to other HIV-related problems, such as opportunistic infections or medication side effects. This makes it more difficult to diagnose and understand.
These are some of the first signs and symptoms of ADC:
Short attention span
Trouble remembering
Poor judgment
Slowed thinking and longer time needed to do tasks
Irritability
Unsteady gait, tremor, or trouble staying balanced
Poor hand coordination
Social withdrawal or depression
I am not saying it is 100% impossible, just unlikely.
Freddie's MO all along was that a song is just a song.

I think Jim Hutton wrote about this in his book "Mercury & Me," fairly sure he indeed said it was about these effects that Freddie was suffering when he wrote the song

A few years ago on Wikipedia (Think what you want I don't give a shit) in the article about Slightly Mad, it was said that a female radio host was told by Freddie that it was about Aids related-dementia. But it disapread from the article at some point. True or false, who knows.

By the way in the phrase/idiom '" I'm not quite the shilling " i thought he was referring to the coin.
In a related to the song forum i found this:

Shilling: I've heard it used as a verb as well and DECK. Both with sexual connotations.
Not quite sure bout the 3 wheels. Maybe there's something regional to that aside from the training wheels tricycle incomplete car thing.

I found that on an old queenzone post but you were part of it so you must know. Seems legit explenation...
"It's a bastardization of "hit me for six", an idiom that comes from cricket, wherein the batsman gets 6 runs for smashing the ball over the boundary without it hitting the ground.
So to be "hit for six" or, in Freddie's words "knocked down for a six" is to be metaphorically struck very hard or bowled over, to use another cricketing idiom. "

Absolutely, unquestionably false. Freddie made no statements about his illness before November 23, 1991 and was in no condition to talk to any radio host afterwards.

Jim Hutton's book makes it sound like Freddie was really enjoying himself (literally hysterically laughing his ass off with his friends) while writing the song.
Always seemed a pretty 'up' and silly track to me, much like a few of the tracks on ANATO. I love it.

Freddie was a very clever, intelligent and educated person. I was able to write songs with cryptic and double meaning (Queen I, Queen II, SHA, The Prophet's song). So what is the main idea behind IGSM, only Freddie knows it. And he's dead.
